Ultimate Therapy is hiring a full-time Occupational Therapist to provide hands-on rehab services in a skilled nursing facility. This position focuses on helping residents improve independence with daily tasks and overall function.

Responsibilities:

• Evaluate residents and establish individualized care plans
• Deliver treatment targeting self-care, coordination, cognition, and daily routines
• Introduce adaptive methods and assistive tools to support independence
• Monitor outcomes and adjust interventions as needed
• Collaborate with clinical team members and communicate with families
• Complete accurate and timely documentation

Qualifications:

• Active Occupational Therapist license required
• Skilled nursing experience preferred
